I am looking for a good annual checklist that covers appendix D available as a commercial form- perhaps in a pad or bi-fold booklet or some other easy to read and use format. anyone know of anything?

You can get a copy of App D online at www.faa.gov, copy/paste that into a Word document and put your N number, a place for the date, Hobbs time, tach time etc in the header, a place for each line item to be signed off etc. Then save it as a template and use it yearly. Consistency is nice!

You can add other enhancements depending on how sophisticated you want to get with it, such as ATA codes, which line items generate the most nonroutines, all that kind of thing. It's not just the airlines who can benefit from that kind of attention to detail!
