This is what can happen if one of the circlips isn't fully seated and the counterweight pin comes out.
This happened on the second test flight of an overhauled O-470 . The counterweight exited the engine making a 6" hole in the case and breaking off the right mag. The counterweight was found lying on top of the engine after landing.
